For me, Fallout 3 made my mind wander to bad places. Let's face it, being the sewers with freaking ghouls breathing heavy in the darkness as you make your way does raise some hairs on the back of your neck!

I have to agree,FO3 does have some disturbing moments, and yes,the ghouls sound bloody shocking, especially as you are trudging through a pitch black irradiated basement somewhere.Great atmosphere

Playing Resident Evil on nightshift with two colleagues, who'd fallen asleep. Walking down a corridor and some birds burst through the windows... almost pooped. Woke the other two up and re-started that section and watch them both poop too.

Played CMR after that.

Fahrenheit aka Indigo Prophecy

And the best "jumper" I've seen was in FEAR 2 (or 1), the room with the pool
in it, the lights go out and if you have your flashlight pointed somewhat level
or slightly downwards as you scan the room, you'll see a very brief glimpse of
the little girl standing RIGHT next to you. Brilliantly executed.

An awesome game. Though I don't remember it as particularly scary. Maybe I've just completed it too many times.

From the top of my head I'd say Dark Fall 1. That game just did so many things right. The sequel wasn't as good.

Silent Hill 3 Was disturbing.....VERY disturbing. very scary too.

Dead space and Dead space 2 were also very disturbing, and made me have aheart attack sometimes when the sneaky bastids pop out of a vent, even though i feel im always ready, there are times when you dont expect it and they burst out tof the vent, or come up from behind.

But Back in the day, when i was much younger, i played Alien Versus predator 2. pretty old game. So i was sent, alone, to restart generators.
As a human, you get motion detectors. EVERY movement shows up. Including the slight sway of a small hook, or ANYTHING. For 30 minutes i hear things rumbling around, get contacts here and there of swaying little objects. saw shadows ect.

The suspense was killing me. And when i actually DID see an alien, i nearly fell out of my damned chair.

Fallout 3 and New vegas gave me some jumps when a ghoul or something comes up from behind.

I got signals. I got readings, in front and behind.

He's right. There's nothin' back here.
Look, I'm telling ya, there's somethin' movin' and it ain't us! Tracker's off scale, man. They're all around us, man.

The first Stalker and the first Silent Hill. I was playing that bit in the hospital at 3am when I first ran into those manky ghost baby things. And that big empty room where the telephone suddenly rings. Totally cliched, totally predictable, totally nearly cacked myself.

I've been running through the marine campaign in Aliens versus Predator 2 over the last couple of days - nothing like the blip of the motion detector for putting you on edge!

Yes! that damned blip around a corner gets my blood going, and i charge in full auto and it turns out to be a swaying crane hook. Then my motion detector doesnt detect ANYTHING when i first see an alien burst out of the ceiling.

The library level in Metro 2033 gave me the creeps...I actually had goosebumps whenever I saw one of those pesky librarians. Gotta get myself a copy of Stalker.

Fallout 3 creeped me out just in walking around the Capital Wasteland; while it wasn't a one-to-one recreation of DC and the surrounding suburbs, it was still eerie seeing landmarks I'm very familiar with in that post-apocalyptic setting.

Hmm... I don't think I've ever come across a game that actually startled/scared me, but it may be that I'm not remembering, either.

It could be the latter
