Most likely a mod conflict.
The original game had this issue and was fixed early on like patch 1.2 or something. Seems you brought it back. As to the leaking pipe you must have had some damage somewhere. Try activating the damage control stations for a period of time. Otherwise there is another mod conflict or something.
