If you rearm and refuel and go back out, the game continues as before. You could, in theory, spend the whole war on a single patrol, as long as you don't get too damaged.

I never get return-to-base orders, except when reporting a contact Comsubpac doesn't want me to attack. The answer to that is usually "woops, too late."

DITTO that statement

i agree with the others sometimes the best thing to do is plot a course to the nearest base and select the refit option and then head back out

I did not see a large drill bit on my last load out... where exactly do you pick one of those up at? <G>

You could, but you won't have anything to sink.

Each succesive campaign layer loads when you return to port depending on the dates. As each layer only has a finite number of targets, if you don't return to port they will not load and you will be all alone.
