Actually I believe the deployment of the ram torpedo went flawlessly, they weren't sunk until much later.

I don't think we will ever know exactly what caused the sub to sink. Could have been human error, could have been damage from the spar torpedo exploding, could have been a design flaw.

Since that was the third time the thing sank, killing everyone aboard each time, I vote for design flaw.
