Keep loosing Crewmembers

It happened again that i lost a officer while on patrol. NOt killed but missing and can't replace. The game says i have eveyone but i am short a officer. It has happend a few time but no more than one officer missing at a time. It makes it a pain to run a ship with one missing.
any thouhts?
Oh and had a great mission with the new mods and boats and when i reladed it it locked up . I ended up having to load the game back when i was in base. I had over 70k in kills and it was a long patrol that i will have to do again! Argh! I dumped the mods i loaded for this mission and kept the ones i had ran before.
