Difficulty Level When Creating New Captain

The game asks you to pick a difficulty level when you create a new captain. What do the various difficulty levels affect, given that the player can also select, under Gameplay, various realism options such as event cameras and whether batteries have unlimited charge.

Basically it presets the options based on your choice , of course you can customize once in campaign . Not 100 percent sure but it may also affect the skill level of the enemy as well.
