DBSM SH4 Speech Fixes

This fixes three things in DBSM for SH4:

1. "Ahead Slow" is now spoken in a normal voice, and only whispered when Silent Running or other times when they whisper.

2. The rhythmic "Thump...thump...thump..." should now be gone. I say "should". In my tests it hasn't shown up, but I can't guarantee it.

3. "Ahead Slow" and "Ahead One-Third" are now in the correct places. There is also a simple file replacement in the readme that will make them show up correctly in the subtitles.
