this is very sad - I remember reading about it when it happened, but even though this article goes some way to explaining it, I still do not really understand - can anyone explain it please?

If I recall correctly the oxygen candle exploded.

A sad occurance but the bravery of those on board HMS Tireless do a credit to their service.
