I Can Never Promote Anyone

From the very beginning (On my third Campaign now) I've never had the option to promote or rank advance anyone. There is always a "You've reached your limit, blah blah" in the tab box for Rank after each mission.

Do I have to pull off higher ranked sailors if I want to promote someone of a lower rank?

I'm worried all my new sailors are going to organize a mutiny if I never give them promotions.

Generally that is true--you have to off load an officer of petty officer to promote to that rank. However, you can promote the other crew up to those ranks. Later in the game you are allowed to promote officers and cpo's but that my have something to do with new boats you get. Not sure about that.
