You need to get to the area and start looking for rescue flares on the water. If you are playing with map contacts on, you will see blue squares on the map. Just head towards them and start looking for the smoke. You must be surfaced to pick them up and you have to get really close and stop.

You lock on them with the TBT or periscope using the "L" key and the little life preserver will be available for you to press on the HUD.

Now they won't stay afloat forever so time is of the essence.

On one lifeguard mission I had, arriving on site triggered the event to happen, but it didn't happen right away. So it may look like nothing is going on but the wheels are in motion. It turned out to be a naval battle and some planes were shot down, but it took a while before the battle started.
The other was amazing. It was middle of the night in the worst storm I has seen in this game. Pouring rain, high winds, nighttime....I guess that's a good time to fly planes, no wonder they ended up in the ocean.

You need to be within a certain distance to trigger the start. If you are zoomed out too far on the map, you may not get close enough. I think it's around 3-5 miles. As mentioned, use the TBT and keep following the clostest smoke plumes. When you get close enough that in the binocular view the smoke fills the screen, the character in the water is clearly visible, and you are near to running over him, go slow and the life raft icon will enable. You need to click the icon before it blinks off. It takes roughly 6 rescues to fullfill the mission. Keep chasing the CLOSEST smoke at high speed. Keep in mind these guys are constantly moving at the same speed as your typical life raft.
