Hmmm. I have Vista, and I haven't had any problems with the res patch.
I do have SH3 installed OUTSIDE program files though, as vista's clever file virtualisation can make things tricky. If the folder is inside program files, try moving it elsewhere, then set up the res fix again. (you should be able to do this without an un-install/re-install, just fix your shortcuts later)
