The Realism Settings for a Campaign must be set while in the bunker (before you launch on patrol).
So, start a new Campaign and choose Realistic setting, and when you appear in the bunker hit Esc and edit your difficulty options as you like (uncheck "No External View"). Those settings will be used for the campaign. You can change them anytime during the campaign when you are in the bunker.

Why don't you try without external cam and real navigation . Its such a game changer .

I'm nowhere near being good enough, it's been days now and I still haven't sunk any ships haha in my multiple campaigns.
