Two possibilities I can think of the top of my head.
1. Are you using the correct version of the patch? There are two versions the US and EMA ones. Depending on which version of the game you have you must use the appropriate patch. 2. Do you have a late version of SH3 that comes already patched to 1.4? Use SH3Cmdr as Jim recommended to check it.
