I think it sounds like a malfunction caused by an idiotic human error.
How's that for diplomacy? I guess I'll just have to turn duds off. I don't really mind having duds on. There's a certain bit of anticipation I like when the torpedo is about to hit... "Is it going to work... is it going to work?" . . . *BOOM!* "WOO HOO!!!" But if I know the torpedoes are programmed to miss every time on certain ships, even with a perfect shot, that kinda takes the fun out of it for me. Duds off for me then. As soon as I get done with this patrol, that is.

You could also edit the torpedos_us.sim. It´s pretty easy.
