Thanks @Propbeanie. Yeah, I think the issue is with FoTRSU. I uninstalled it and installed TMO BH with OTC and everything works as it should...other than random crashes. This is with 2560x1440 in game settings and 3440x1440 windows resolution and the OTC patch for 16:9.
