Working with KFG we discovered the source of the TLAM and insertion error messages. It is described here:

If you notice the fix is to adjust settings in the campaign files. I believe this means that any modder making their own campaign will also need to use the revision in their mod.

Since the fix isn't in the game engine but values in the campaign data files I suspect if you don't apply the new numbers to your modded campaigns you will end up having the bug.

Wanted to do my part in letting the modding community over here know that.
