Classic US Navy Training Film

Found the classic 1975 US Navy training film "I Relieve You, Sir" on Youtube the other night. One version is located here:

Having seen this during my early naval career as well as learning the details of the 1975 Belknap-Kennedy collision, the lessons presented here were always foremost in my mind before I took the watch as OOD.

I wonder if any players experienced this sort of "Loss of Big Picture Accident" while playing multi-player scenarios.
