Hi; I have been wondering for a long time what effect having the captain killed or disabled has? Is there an accuracy penalty of some sort?

Yup, when the original skipper buys it, the ship's reacton times go down somewhat. Also, if the ship has a director, conning tower hits tend to knock that out so the ship reverts to less-accurate fire control.
