I used 50 nm (so far only had 1 patrol, just finished my first career patrol) and it worked. Using TM 1.7.4

well, i'm new so i'm really having a huge WTF. i got a patrol mission to the island of honshu and when i got there i had to follow a set of waypoints ( so nothing like staying in a radius or something) and i did that. and since i'm new i read in the manual that you can complete patrols only if you return to your homebase, so i did.... guess what: mission not completed yet. WTF? help plz

Patrol.
This mission requires only that you stay within 100 nautical miles of the objective for 48 hours. You usually get 200 renown points for completing this mission.
It doesn't care if it gave you something else to do instead - it only cares that you stay there for the time required. Dumb, ain't it?

WELCOME ABOARD!

With patrols, is it 50nm and stay surfaced? Do I have to move around the whole perimieter too?

No need to move. You can even dive, if need be. Just don't wander out of your assigned area. Naturally, if an unescorted, large tanker lurks just outside that 50nm radius, you can always do what you're compelled to do ... then return and start the patrol over again.

Your call.

okay. I've been within 50nm of my objective in Honshu for thre days now. Think I'll just dive right over the spot with unlimited air for simplicity's sake.

It would be more accurate to say that your "patrol failed" ... meaning, yes, you need to return to base. Or, you could send a status message ... then you'll probably receive orders to complete a second mission.

Until you RTB, you'll continue to see that "failed" message every time you save the game.

It doesn't mean anything so long as you save before you quit.
