On behalf of Halsey (I no longer play online)...
Faulty depth settings.....The torpedo tended to run some ten feet (3 meters) too deep for several reasons.

Magnetic exploders, depth keeping (calculated for practice warheads which are much lighter than real warheads), contact detonator pins bending (fixed by switching out steel for aluminum), circle runners (mostly mk 18 due to cheap and quickly built versions, killed the Tang)
