No, the patcher is not rerun unless their is a new snapshot for it.
Just add the mods when docked.
The important thing to look out for is whether you need to to start a new career.

Sobers mega mod current version has mods that i think enhance the real navigation feature .
Blackout lights : darkened towns with no or very few lights .
Darkened harbor walls .
No harbor pilot .
No fluorescent sand on beaches .
Entering Kiel .
