Then it's probably a bug, either with the tube not loading or the tube not showing as destroyed.
Personally, I'd assume the tube has suffered a major malfunction that rendered it inoperable, and the parts are not in the ships stores so it can't be fixed until the boat has returned to port. Run without tube 1 for the remainder of this patrol, then when you start your next, fire tube 1 in a safe direction and see if it reloads. You can always refit to regain that fired torpedo.
