Yup, it happens. Sometimes for who knows what reason the random song becomes the same song over and over. You also get a few play one song, play song A, play another song, play song A, and repeat until you can't stand song A.

I've noticed that SH4 doesn't like different file types or different forms of audio compression in the same folder. Check if the music files in the folder all have the same compression (e.g. mp3 stereo 44000 bitrate)
