And now it works, but it's still skipping some options, like no intro movies and starting out on sea.

Are using Windows Vista or 7 with the game installed in the Program Files directory? The more recent versions of Windows are very protective of anything in Programs or Program Files, I wonder if it would be keeping Commander from making some of the changes necessary to implement your selected options.
