If I then exit back to desktop and run Task Manager, the first attempt to run still seems to be running (SH3'something'.exe if memory serves), apparently in the background?
I've tried every possible combination of compatability modes and running as administrator but always with the same result. Does anyone have any ideas?

What is the exact place you installed it into?

You should not get a prompt asking if you want this program to make changes, unless it is in Program Files, or you configured it to run as administrator on that compatibility tab of the shortcut.

Rename sh3.exe to SilentHunterIII.exe.

What difference does that make?
