TC and sudden damage to your uboat.

Aight, so i use TC.. quite alot.. i imagine we all do.

Last night while using TC, i was suddenly informed that my boat was taking damage and
all my crew on the bridge was wounded (both the crew on watch and the 2 crews off watch), so i initiate repairs, looked for anything that might have attacked me, scrathced my
head and continued my voyage.

Sometime later, im at periscope depth (cannot recall if the periscope was up or down), and have been for several hours during daytime,
and using TC again.. when suddenly im informed that my boat is taking damage and all my crews (both on and off watch) in the engine room department are wounded. I again initiate repairs, surface and look for anything that might have attacked me. again nothing.

question:
1. why are both crew on and off watch wounded, shouldnt those off watch generally stay out of their work stations ? (be it engine room or bridge)

2. has anyone confirmed if theres an actuall attack by something (plane or ship) when one gets damaged when using TC or if its a "screw up" in the game itself?

any and all help appriciated.
