Default RN Reports from Boards of Inquiry

I came across the published boards of inquiry reports for various RN accidents, including a number of submarine ones, which I hadn't seen mentioned here before and thought may be of interest to some. They're edited to de-classify them but still make interesting reading from an operations point of view, particularly the narrative parts:

The explosion on HMS TIRELESS that resulted in the tragic death of 2 crewmen. I found the narrative on this one particularly interesting, and quite moving in parts.

Grounding of HMS TRAFALGAR.

Grounding of HMS TRIUMPH.

Grounding of HMS VICTORIOUS.

I've heard nothing new, other than what's already public. You'l have seen she was decommissioned a couple of weeks ago, but she wasn't fitted with the systems I work with so I had no reason to deal with anyone onboard.

Seamount in a well surveyed area=probable court martial to me,unless she was out of usual transit area doing something unmentionable.

I agree, those grounding reports listed above all relate to human error, mainly mis-communication, if it was an uncharted formation we'd have heard by now I think - they'd release it to negate the bad press a collision brings.

Sheesh, I can only imagine what was going through the mind of the CO of Tireless when the fire and flood alarms went off after the candle explosion. The two worst possible things in a sub, and extreme kudos to the injured crewman who was able to deal with the small fires that started after the explosion.

Yeah, he did an amazing job. That escape flat's not somewhere I'd have wanted to be stuck for 3/4 of an hour - that must have been terrifying.
