Sometime it happens

Hallo, it sometimes happens two strange things:
1) Sunk but not counted: normally is is registrered that the torpedoes has reached the tgt and the crew enjoys, but ... you don't read the ship as sunk in your board
2) tubes on the rear: says the the personelle is not enough, even if it is not true by reading the green line on the sector, but the torpedo is reloaded anyway.

I use a cleaned copy of SHIII with mod GWX 1.03.

Ciao

yes I have seen this too - I recently sunk a destroyer escort, torpedo hit - she went down - crew cheered, but no sunk plotting on the map nor any credit in the captains log book of the kill.

Unfortunately I have no idea why either

This is a stock bug and has nothing to do with GWX. Do you see the red icon on the map? To get cerdited a ship sinking you must be within range (visual/radar if on the surface, hydrophone if submerged)

Stock bug as well, but in this case everything seems to be working fine. The only thing that is wrong is the message you get.
