Lots of testing. I set up a test mission with the Mark 14, and I used the above-listed dud settings. Result: the torpedoes explode every time without fail. When I set the dud rate for entry 0 to 100 percent, the torpedoes will fail to explode every time. I will re-run my tests again, but I'm almost certain my conclusions are correct.

very interesting luke, but could you also cite the reference for the specific pistol failures?

The Osprey book on the Type VII mentions it, along with David Miller's U-Boats: The Illustrated History of the Raiders of the Deep.
