Hopefully there were no elderly people on the train!! Not good!!

Happens, sometimes on purpose. Johnny Buchardt did this during the Cologne Karneval of 1973 (important things happen during the first 20 seconds):

I understand that mistakes can happen.

I am, however, having difficulty understanding how a recording of Hitler can accidentally be broadcast.

You can ban the Nazi party, ban the Nazi propaganda, and ban all symbols of Hitler’s Third Reich, but the one thing they cannot do is ban the ideology behind it all.
