submarine combat patrol insignia
I am sorry. But my google-fu has failed me today, or maybe I am not looking hard enough. However, does anyone know when the last time this beauty was awarded?

Well, I have nothing to back this up, but I'm going to place my bet on the USS Torsk (SS-423) for her 2nd (final) war patrol. She sank the last Japanese ship sunk in WWII on August 14, 1945. I know that the award of the submarine combat patrol insignia for this patrol was authorized in the patrol reports dated September 16, 1945.
Just what came to mind first, don't know if it's the correct answer.
