start campaign... & BOOM!!! wtf?

i am not really sure wtf is going on here. plz help. i start a campaign thru SH3 Commander for 1942 @ 29th flotilla in the mediterranean. i start well. go (start engine) and try to sail out slowly (ahead slow) and a few minutes in i just go to %^@#!$ everything goes to %##$$!@ like one of my forward torpedoes have exploded causing massive flooding and death!

i immideately paused to at least figure out what happened... as far as i can tell i was far from shore (it was late evening/night), there was no traffic in front of me. (i think).

i've read that sometimes stuff happens due to "sabbotage". is that true? one time my "ahead flank" didnt work due to this, i think. can this happen too? not really, right?

this happened 2 times in close to same spots near there port in italy. can you help?
