difficulty level setting?

Hi, returning back to the game after an extremely long break.
How do I set my campaign game level to my settings?
on main screen difficulty I set it to 100% realism. Then turn on exterior camera mode, and turn on map contact & updates.
Yet when starting the game it gives me another realism option that are pre-set and cannot change to what i set in the main menu.

How can I change my campaign game game difficulty to what I want?

When you start a new campaign, you have to set a difficulty level, which is "Easy", "Medium" or "Realistic".

This changes the settings for the campaign and not the general settings of the global game. You can, however, adjust the difficulty level in detail of the campaign when you are in bunker with your boat.

PS: This is for TWoS and I cannot speak for the stock game, because, I don't played it.
