Default I can't make difficulty settings stick?

I want to play a campaign with external view and map contacts on.

I go into options and set this, and hit apply changes as a custom difficulty setting.

But when I go to start a new campaign, it defaults to "realistic", and when I play the game I can't access the external camera.

What gives?

I'm confused.

When you start the game, you start off in the bunker.

This is where I go to options and set the realism to turn on automatic map updates and turn on external views. This gives a realism of 84%, and I say "accept changes".

But then when I start a new campaign, these realism settings do not seem to be taking, and I have to choose another setting.

Ah I see now. After you start a new campaign and are in the bunker you have access to the gameplay options again.
