Sounds like your computer is a bit slow. How about posting the specs and we'll see if anything can be done.

There's an old post somewhere about how the number of frames your game looks ahead (one of my nVidia settings for vertical sync is set to "Render no more than 3 frames ahead") can affect the speed of your cursor.

Good to see you got it fixed.

You will still experience erratic mouse movements on the map screen at high rates of time compression. There is no fix for that other than reducing the TC.

Yes strange mouse behaviour IS connected to Vsync. I have googled this problem a couple of times, and all the poor guys were getting told to upgrade their GFX-card.

I have a OK GFX card and SH4 is silky smooth. But I sometimes experienced strange mouse behaviour. (Not jerky but kinda slow and accelerated at the same time). I have installed/modded SH4 quite some, and sometimes got this, and other times not.

If I turn off Vsync the mouse is much more normal. I am glad to know that now, and I hope you guys "spread the word". Or maybe disagree with me. Thats fine too!
