where is No noise meter?please help!

Hello to everybody. i am a young captain( never play silent hunter before) and i want to play with a little realism.
When i go to Game SETTING i cant find No noise meter.( i try to play 0 realism but the icon during the game is missing.there isnt!!

) i patched the game but still miss that option.

and also my sub is incredibly fragile, often two depth charge and i sink in a few second. is it a bug?
Anyway i cant find no noise meter in the option , same of you can help me?( i am not english sorry 4 the gramma
