I play career - high realism, DID.

Sometimes I will use missions for testing/modding purposes, but that isn't really 'playing', as such.

Career when I get serious, single missions for fun. I don't play DID unless it's during an attack on a TF. I have limited battery/fuel/air/CO2 on. Auto torps and dud torps on, realistic sensors off and map contact updates are on. My mod setup is TMO + TMO Small patch + RSRDC for TMO + RSRDC Patch V5xx and anything else is just something i'm in the mood for.

Career: TMO DID, External cam only, some times no duds.

I'll play single missions for fun, will load up most anything including Rocken Robins slightly sub nuclear torpedoes.

My policy is to NEVER alter game files, so that I can easily dump the mods and go back to stock as a starting point if I ever get a CTD or other bug that I can't work out otherwise.

I also tried, and didn't like, the "randomizer" as it does alter game files and they can't be easily restored. So I slapped a couple different image files from Fred8615 together to get what I wanted and rolled them into MODs for my use.

As far as I can tell, I don't think I'm really altering the game play. Maybe the Ship Maneuvering mod - but even DDs don't take off like racing boats so....

Haven't played in months, but I play campaign, but do about 5 patrols and start over. I play cams off, contacts on...feel a fair balance.

I use TMO, my own version of RSRD, parts of Travs mod, my own envs and a bunch of tweaks..

That's a given as far as I'm concerned, especially with SH4, which allows you to run over islands that don't even show up on the charts. In real life your crew would see it.

My Dead Is Dead constitutes anything I consider realistic. If they kill me, I'm dead. If I'm running at high TC and they appear out of nowhere kill me before I can even react, then the game cheated.
