With its budget getting real tight, they need to spend this much on conferences?

Money should be spent on caring for the Vets themselves.

The question I have is: Where is the spending oversight/review for these government agencies. In the US Government, everyone reports to someone. How come this person/committee is not saying "uh, no. You can't spend 4.5 million dollars on a conference."

When I was a govie, there were no shortages of people telling me what I could not spend and I was not spending anywhere close to this for a conference.
