It's mentioned in the manual, but I can't find it either in the HUD or realism settings. I have difficulty on hard - is that why?

If it's mentioned in the manual it's probably due to poor editing or being overlooked as the project progressed. There simply is no noise meter in SH4 telling how high a profile you're presenting to escorts like the one in SH3. Even is SH3 it was regarded as something of a crutch as no such device existed.
