Reloading torpedoes

Why do torps sometimes never load? In the reload screen its permanently in the torpedo waiting in queue to load. I have a full bow torpedo room with the torpedo officer there as well and still there is no loading. Autoload is also ticked.

I'll start with the most common hang-up. Are you at silent running? Can't reload torpedoes without making a racket.

Here's another tip. Make sure the door of the empty tube is closed if you are submerged. If it's open then that tube won't reload. I've accidently done this before.
