I know you can get new subs and be awarded medals. Can you be promoted to Commander?

Yes, but you're not told so in game, nor does it have any apparent influence on crew/boat performance afterwards. You can check in your save game folders, in the CAREERTRACK.UPC files to see your current rank (top of file, first block of data).

They do tell you when you get promoted, you'll find an entry in the captains log after the patrol.
