Save Game????

For some reason, while playing in career mode when I go to save game, it asks me to type in a name for the save game and when I go to load it isn't there.... not only that but my last mission I got back to base and ended patrol and it suddenly said I was dead in my save game list. career gone. WTF is going on??? Anyone know of this or experience it?? Any help would be appreciated, starting career for third time now
