your ordersbar.cfg works correct on my system (only problem with the broken words in your textcopy)
1280*1024 on a blank SH4 1.4
I've made a test file for you. It uses another Iconfile. only unpack in the mods folder and run it via jsgme.

I cannot imagine why you and fred get this trouble with the icons. They are well defined in the ordersbar.cfg.
And both ordersbar worked fine on my system.:hmm:
