Anyway once you get it running you can enable any of the options listed. There are descriptions for each of the options.
I guess this then updates (patches) the executable, though I don't see any button that says "patch the executable".
So I guess it just does it when you set the option(s) to enable.

Exactly...Or you can select
file and then
restore snapshot option which will do all the job for you. A snapshot is .gps file which contains all predefined settings for the patcher...
