I honestly tried to answer 37 of your questions.
It is up to Guam and Puerto Rico to decide whether they wish to be part of the US. It is not my decision as a John Q. Public voter -- the islands should declare their intentions to be recognized as states and therefore be allowed in.
I'd never thought of Iraq or Afghanistan as US states. Interesting questions. Why should we consider them to be US states? Do they wish to be part of the States? They should incorporate on their own if they so choose, per Guam, Puerto Rico and so on.
In regards to your final question, I must ask you my own:
Would the world be better off with the US' system of government? If so, what makes our system superior to any other current political system in the world? These are loaded questions, but fair is fair. Nothing is permanent in this world.
