1. No, as far as the game is concerned they are in whatever 'compartment' the crew management screen shows them in. Even submerged, the deckgun crew is still on the deck (even though they don't show when submerged) That is why some mods created crew compartments to put them in when submerged, for example "Hogan's Alley". So if the engine compartment shows all three watches, as far as the game is concerned all three are in the engine room. And the bridge crew is still on the bridge and the guns crew are still at their guns.
2. Like the others have said it is most likely a plane. Your sonar "usually" picks up a ship and drops you out of TC. But a plane is not detected and while the bomb is dropping the plane is still flying away at "high TC". So by the time you get the damage, drop out of TC, check the crew, surface the boat, the plane is long gone. The next time you get attacked on the surface, time how long it is until the plane is out of sight. It isn't very long. A lot less time than it takes to surface and look for him. How many times have you been on the surface, alerted that a plane is coming, know the direction it is coming from and still can't see it?
It can also be the other things mentioned, but if you zoom in on the map, you should be able to determine if it was an atoll or island.
