Its not all flat actually
Try adding a couple of ports and you will see what i mean
True it doesnt have the vast changes there are in RL
There was a terrain editor around a while ago - not sure exactly what you could do with it tho as never used it
Try a search of the forum or even over at uboat.realsimulation

No, its not all flat, the rock of Gibraltar, for instance.

I just know that the islands of Lofoten on the norwegian coast isnt flat. When im sailing in the "Vestfjord", heading for Narvik, i would like to see those lofoten mountains... And as stabiz is telling us, Gibraltar isnt flat, but not just like the real Gibraltar....

the only problem making mesh would be to make the aircrafts avoid the mountains.
( consider FS9's AI traffic with mesh...)
Mountains would make it more realistic, when the aircrafts had to take those mountain sides into consideration during an aerial attack.
You could also use fjords for hiding, not only from aircraft, but DD's and other bad surface vessels..

Well the aircraft do have a section to set altitude
But the AI routines are crap so they would prolly fly into them anyay - as the DD drive straight into ports
