Playing the campaign for the first time under 1.09c, I stumbled on a possible outcome that doesn't seem to be accounted for in the campaign. From the looks of it, your ship being lost with all hands autofails the campaign. You get the string of newspaper articles depicting western Europe falling to the reds and whatnot regardless of the circumstances of the result of your final mission.

I was sent to infiltrate the bastion and sink the Typhoon. I managed to sink the boomer as well as two out of three Alfas escorting her, but she counterfired and sank me too. I was too deep to abandon ship. I got a posthumous medal for completing the mission, but still failed the campaign.

yeah, but it would have been nice if it still counted towards the NATO victory we came so close to.

It is possible that since you died, NATO only got information from your subs rescue beacon. No way for you to tell them you sunk the boomer. Soviets bluff at the negotiations was still viable.
As far as a gameplay bug, don't know.

not saying it was a bug, just a possible oversight. Although I get what you're saying about NATO not knowing that I succeeded.
