Strange situation I didn't expect I must have been to excited and shot from the wrong angle. Two of my torpedoes shot directly into the hull of my boat and killed me....

Yes, Situation Awareness is important. Did you make them active straight away?

You must not set a weapon to activate unless it is at least 4kyds from your boat in my book, if the wire breaks it might find you instead of the intended target.

Just got this game and havemostly only done training missions so far. Though I did try one single mission for a little while. Didn't know there was another sub in it. He ended up blowing himself up just like you did! LOL Then I proceeded to die from the escorts...

I had one torpedo detonate before leaving the tube on an extreme angle shot.
That's what I get for activating it before it could leave the tube.
