In the settings you have Malfunctions and Sabatoge
What exactly do these do in game ... what malfunctions and what could be sabatoged :hmmm:

Issues due to malfunction and sabotage would occur randomly and from what I understand (I've yet to use the feature) would typically involve things on your boat breaking down unexpectedly or simply not working from the get-go because they either just break, as things sometimes do, or were tampered with in the shipyard.
If you are really curious about the types of things that might happen, go to your Commander installation and look in the Cfg folder - there's a file there called Randomised events.cfg... scroll down a bit and you'll find a secton for MALFUNCTION SETTINGS and even further down, one for SABOTAGE OVERLAY. Those sections contain details on what *could* crop up on any given patrol if you have the feature enabled.
If you enable the feature it's recommended that you test all your systems as thoroughly as possible at the start of a patrol, before you get deep into enemy territory and find out that some vital bit of equipment is not working properly.

you can read all the options (malfunction + sabotage) in the file called "randomized_events.cfg". In the folder "SH3 Commander/Cfg".

Are you informed someway by the crew or do ya have to figure it out for ya self ...
And if ya have to figure it out for ya self is it possible ( I don't have the knowledge ) that someone could make it so that you are informed in some fashion by the crew as would have been in real life

I think in most (if not all) cases, with the way the feature works, you just discover that something isn't working the way it should. I suspect that much of the "damage" is simulated by altering the files that control the specs for your boat and equipment (for instance, decreasing the max range to simulate a fuel leak, or something similar), so it's not like when you start out with a fully functional boat and then get damaged and the crew notifies you that there's (suddenly) a problem. But I'm not sure since I've never used it myself.
